Admission

Eligibility for admission to The Northfield, an enriched living community, is based on the following criteria established by the New York State Department of
Health. All equal housing opportunity regulations also apply.

• Personal interview with the Administrator of Assistant Program Director within thirty (30) days prior to admission
  
  
• The Northfield is unable to admit applicants or retain residents who require continual medical or nursing care or supervision as provided by an hospital or skilled nursing facility; have unstable medical conditions; have severe behavior problems; who refuse or are unable to comply
  with a prescribed treatment program (including
  but not limited to a prescribed medication regimen); who are chronically bedfast; are unable to transfer without assistance of another person or requires the continual physical assistance of another individual to walk; have chronic unmanaged urinary or bowel incontinence; is not self-directing (requires continuous supervision
  and is not capable of making choices about
  his/her activities of daily living); engages in alcohol or drug use which results in aggressive or destructive behavior; are a danger to themselves or others
  
  
• Additional functional criteria:
  
  
  • Applicants must be able to independently perform their daily hygiene and dressing routines with minimal staff assistance and verbal reminders
    
    
  • Applicants utilizing wheelchairs must be able to propel them without assistance
    
    
  • Applicants must be able to feed themselves and are able to remember to come to meals at scheduled meal times
    
    
• Behavioral criteria:
  
  
  • All applicants and residents must be able to follow verbal instructions and accept direction and cueing without agitation or hostility
    
    
  • Applicants/residents must be aware of their own floors and apartments, and able to distinguish their personal property from that of others
    
    
  • Applicant/residents must not be a danger to self or others
    
    
  • Applicants/residents must not disturb or interfere with the care and comfort of others
    
    
• Administrative/Paperwork Criteria:
  
  
  • Submission of the completed application form
    
    
  • Submission of financial documentation with required verification documents
    
    
  • Submission of supporting documentation, i.e. completed medical form
    
    
  • Payment of refundable application fee
    
    
  • Payment of security deposit
